If you are applying for an apprenticeship, please also submit a Recommendation Form from a reference.

First-time Costume Apprentice and Costume Staff Stitcher applicants must also include a hand sewing sample which includes examples of the following: cross stitch (herringbone/ catch-stitch), whip stitch (hemming stitch), slip stitch, hook and eye, a 4 hole button sewn with thread shank, and a snap.
